Chuck Jacobson maintains the UFMDC Research and Clinical database called INFORM. This IRB-approved database is used to gather information on our patients’ disorders, how their medical and surgical treatments are helping them and various demographics. The power of the INFORM database comes from the multidisciplinary nature of our team and the fact that nearly all of the patients who visit our physicians want their information to be used to further our clinical research. In 2009, we will have over 4000 patients with a variety of neurological movement disorders in the INFORM database.

Chuck supervises ten pre-med volunteers and three part-time staff who assist with data entry, preparation of scale and questionnaire packets for clinic, and editing of videos from clinic visits. He also measures the locations of patients’ deep brain stimulation leads using the same UF-developed software that Dr. Foote uses to find the initial target in the operating room.

Chuck developed this website for the Movement Disorders Center and now maintains and updates it.
